This is a good question, where I fear you may not get exactly the specific example that you want. I understand what you are asking, where I served in Armor/Cav. I do a lot of project management in my civilian life.

Project was to move xxx size unit with $xxxM of heavy equipment xxx miles for required training and evaulation that spanned xxx months. Responsible for xxx scope. Owned xxx portion of the initiation phase. Planned xxx and yyy, which was executed flawlessly and recognized by being awarded xxx medal. During event, was responsible for monitoring xx and controling xxx, where I proactively initiated xxx to ensure control. Effectively transitioned back to home station, where I owned xxx scope to ensure effective closure of the training effort.
I would then repeat the general format for the other two positions, where I am using the phases to build the sentences of the paragraph.
