I would also suggest that you contact the nearest Disabled American Veteran's group. They will advise you, guide you and help you fallout the necessary forms

Dear Joe,

You can apply for VA medical care, online, at www.va.gov/healthbenefits/apply/. As noted on that webpage, you may request a doctors appointment at the time you apply for enrollment. Please click on the hyperlink under "What happens once we receive your information?" for additional details.

All of your service-connected and non-service-connected medical care is will be free; your medications will cost $8.00 each, unless you submit a "means test" that show your income below some threshold, around $13,000 (it varies geographically, and by number of dependents.) If you have insurance, the VA will first send the bills to your insurance company and then cover whatever amount your insurance company does not. Again, the only bill that you will receive from the VA is for your prescriptions.
